Online with a Credit Card: Our online application form allows you to enter your membership application electronically via a secure, encrypted link to our server. A credit card and e-mail address are required. Online memberships are honored as of the time submitted--24 hours a day 7 days a week, pending receipt of payment from your credit card. If an online application is made on on May 1st, the membership will begin on May 1st. An individual may join PMA up until the time the funeral home is notified of the death. Please note that the online application process is not completely automated. You will receive an email after we have processed the application in the office. Online applications are processed about twice a week.

By Mail: If you have one of our membership brochures, fill out the membership application form inside and send it to us with a check or money order for $25 per person. Or, you can print out our membership application (PDF) and mail it to us with your check or credit card information to:

By Fax: You may submit your membership application (pdf) by fax with credit card information. Our fax number is 206-529-3801. Fax memberships are honored as of the time received, pending receipt of payment from your credit card. This Fax machine is in a secure location and completed application will be locked in secure location.

In Person at the PMA Office: If it is convenient for you to enroll in person, stop by our office, located on Capitol Hill in Seattle between Madison & Denny on 12th Avenue. Our address is 1801 12th Ave Suite A (map/driving directions). We are open Monday-Friday from 9 AM to 3:30 PM. You may pay by cash (exact amount only), check or credit card. We are unable to make change for cash transations.

By Phone: Membership applications by phone are accepted during business hours only if death is imminent. A credit card number will be needed. Our main number is 206-325-0489 or outside the Seattle area: 1-866-325-0489. Office hours are Monday-Friday 9:00-3:30 pm.
