When Death Occurs:

Family to choose which funeral home from our list of more than 20 PMA contracted providers.

Call 1-888-PMA-2PMA (1-888-762-2762) to contact the PMA-contracted funeral home.

Funeral home will pick up the body.

Make an appointment to meet with the funeral director and complete arrangements.

Bring to the funeral home any completed funeral planning forms such as: Disposition Authorization or Putting My House in Order.  Also bring military discharge paperwork if wishing to access VA benefits.

Make payment arrangements with funeral home.

Death Certificate must be signed by the physician and filed with the medical examiner before any cremation or burial may occur.

If cremation has been chosen, pick up the urn from the funeral home when cremation has been completed.
